Long Term Disability Insurance

The Catholic Principals' Council of Ontario (CPCO) offers a group benefits program that includes Long Term Disability Insurance, Optional Life, Dependent Life, Accidental Death and Dismemberment (AD&D), First Alert, LifeBalance® (employee assistance program) and Critical Illness Protection.

The CPCO Benefits Programme features:

• 5 choices for Long Term Disability coverage, all of which include the First Alert, LifeBalance®, and Critical Illness Protection benefits.
• Up to $100,000 of Optional Life insurance is available for you and/or your spouse with NO MEDICAL QUESTIONS asked if you apply within 90 days of appointment! An additional $100,000 Optional Life insurance is available with medical evidence.
• Accidental Death and Dismemberment coverage, of up to $200,000 for yourself and/or your family.
• Up to $20,000 of Dependent Life Insurance is available for your Dependent Children with no medical evidence if you apply within 90 days of appointment.

It is the member's responsibility to contact Johnson Inc. for an application package. Newly appointed members have 90 days from their date of appointment to join the plan without medical evidence. If you have not been provided with your CPCO membership number at the time you submit the application, please indicate on your form that your membership number will follow. Once you have received it, please forward your membership number to Johnson Inc.

Enrolment Criteria:

• You must be a member of CPCO and your principal group must be participating in the programme.
• If your principal group has not had a presentation by Johnson Inc., please speak with your association executive to arrange for a presentation. Your principal group will have 90 days from the date of offer from Johnson Inc. to join without medical evidence. The date of offer is set between your principal group, Johnson Inc. and your school board, after which, members will be contacted individually by Johnson Inc. to join the programme.
